Notes:	You MUST bring your driver's license and CPR card; otherwise, you will NOT be allowed to enter. If you are coming from another institution, you MUST bring your OUTSIDE CANDIDATE FORM signed by your EMT Training Institution. You will not be allowed to enter without this form. Please arrive 30 minutes (07:30AM) prior to the exam start time. Please bring your own non-alcoholic beverage and a light breakfast. Please have a patient. If you do not have one, let the chief examiner know that morning. Leave all study notes, station sheets, and tablets in your car. Your cell phone MUST be turned off and MUST NOT be taken out or used at any time during the exam; otherwise, you will be removed. Wear comfortable clothing. NO open-toed footwear for the patient. No Fire/EMS apparel may be worn during the exam, for either you or your patient. AD Quality EMS Training Inc.
